The UPS Store, 11 South Angell Street, Providence RI, and
Who agrees as follows:
1. The UPS Store agrees to store those boxes marked numbers (s) for
a _______ month period, beginning _______________and ending
___________.
2. The UPS Store agrees to rediliver the contents of storage to
student for a $25.00 redelivery charge. (Curbside Only)
3. The Student has on this day prepaid to The UPS Store $__________
as payment in full for storage services only, until _________(Date) at
the latest. If the student does not claim the contents of storage by
that day, then the student shall pay $5.00 per day per box for
additional storage. Payment for additional storage must be made as a
condition for the release of the contents of storage. If by
__________(Date) the student does not retrieve the contents for
storage from The UPS Store at the address set forth above, then
The UPS Store shall send a notice to the students contact address
above by certified mail with return recipt requested. Notice shall be
deemed to have been made 3 days after placement into US Mail, postage
paid.
4. If the student does not retrieve the contents of storage within
15 days of above notice, The UPS Store shall donate the contents
of storage to charity or otherwise dispose of them. THE STUDENT
EXPRESSLY RELEASES The UPS Store FROM ANY LIABILITY FOR CONTENTS OF
STORAGE AFTER THAT DATE.
5. The student is prohibited from storage of any hazardous
substances of any kind, including but not limited to gasoline,
cleaning solvents, aerosol cans, paint, etc. The student releases The
UPS Store of all liability of damage for the students violation of
this paragraph.
6. The student acknowledges that s/he has either packed the
contents properly for storage, or has paid The UPS Store an additional
fee to properly pack contents for storage. S/he releases The UPS Store
from all liability resulting from packaging errors for contents should
the student decide to pack the content themselves. The student also
agrees to release The UPS Store from liability occasioned by causes beyond
the control of The UPS Store, its agents or employees.
7. The student may authorize The UPS Store to release the contents
for storage to a third party providing the student provides written authorization
to The UPS Store.
